Form is required with all renewals. Choose one from below:

Regular or Lifetime Membership (PDF to print** and fill in- PLEASE PRINT CLEARLY)

Above form in Word format with text boxes (to fill in using Word)

If you have wish to join or renew for a current year, simply strike thru the “2020” and mark it for the current year.

** - If you are going to print the pdf file, then actually DOWNLOAD the file and open with whatever PDF viewer you use, such as Adobe Reader/PDF Viewer, also called Acrobat, or Foxit’s PDF viewer (recommended).
Don’t try to use a print function in your browser, as they rarely work correctly unless using the plug-in’s print command.
